Luxe Home Health is a growing company where Office and a Field team working together to provide excellent quality of care to our patients and support patients recovery in a comfort of their home. We are currently looking for an Care coordinator/Intake coordinator to join our team.

The person we are looking for must have a medical background and be able to read and understand medical records, verify patient's eligibility for Home Health Services and process multiple referrals in a fast-paced environment. This position requires good computer and software skills, ability work independently, prioritize tasks and communicate closely with our office and field team members.

Responsibilities:

A. ADMISSIONS:

-Process incoming referrals, run INS plan, location and review clinical documentation.

- Make a pleriminary decision whether the patient is appropriate for Home health services and could be accepted.

- Make sure all admission documentation is received and attached to the chart according to the Compliance regulations and requirements.

B. CARE and THERAPY COORDINATION:

- Make sure visits are scheduled for clinicians and patients are seen according to the approved frequency

- Attach incoming documentation to the patient's charts

- Provide Daily and Weekly census reports

Required Skills and Qualifications:

1. Medical Background as LPN, RN, Physical Therapy Assistant (PTA), Phlebotomist, Medical Assistant, Medical Coder - preferred.

2. Ability to manage multiple tasks and still maintain friendly environment in the office

4. Detail-oriented and highly organized

5. Strong communication skills

6. Ability to solve problems, create professional relationships with colleagues and Referral sources

All responsibities will be performed in a friendly envirenment of our office and well supported by managment team, who is always available and willing to provide you with required training, advice and help.

Job Type: Full-time

Pay: From $20.00 per hour
