Healthgram is in need of your help. We are currently seeking to hire an experienced ANP/FNP or PA to provide 40 hours per week of on-site clinic care for our client in Norlina, NC. We specialize in providing corporate wellness services designed to lower the health care costs incurred by our clients across the nation. We do this by strategically improving the overall health of a given employee population through efforts such as disease management, on-site acute care services and targeted patient education.

In addition to performing episodic and acute care, our goals are to identify and target “High Risk” employees through annual health screenings, assist employees in making healthier lifestyle choices, and manage any existing conditions. We create and implement initiatives that promote wellness and encourage employees to make a personal investment in their health (such as establishing a relationship a primary care physician, medication compliance, smoking cessation, etc). Sincethis is a workplace clinic, OB/GYN, GU services, and all pediatric services are strictly excluded from our scope of practice.

What you can expect from this position:

• Medical oversight will be provided by a local physician in conjunction with our medical director
• All member encounters will be documented in Healthgram® EHR, our proprietary care management system, in order to manage patient care more efficiently. EHR will also provide the selected clinician with access to integrated medical claims, Rx data, labs/screenings, and self-reported personal health information
• Population analysis reporting will be provided quarterly to assist in identifying high risk members who demonstrate a need to utilize the clinic
• Competitive compensation with a $5,000 sign-on bonus (terms apply)

Requirements:

• BSN from accredited 4-year institution
• Master’s or Doctorate Degree in Nursing
• Graduation from an accredited NP program
• Active licensure in NC or eligibility to obtain licensure
• National certification from an appropriate credentialing body

The ideal candidate will exhibit the following characteristics:

• Be accustomed to working in an autonomous environment
• Have a background in primary care, family medicine, or internal medicine
• Experience in wellness and behavioral consulting is a strong plus
• Be action oriented and solution driven
• Be passionate about implementing life-saving, quality enriching solutions
• Occupational Health experience is a plus!
