Job Summary

Amazing Care Health Services, LLC is seeking a dedicated, patient driven Physical Therapist who is able to evaluates patients and administers customized treatment plans to relieve pain and restore or improve function and mobility. The PT provides quality care to facilitate patient goal achievement and effective reintegration into the home and community.

What You Will Do

• Administer physical therapy screening, evaluation, and treatment for all patients

• Perform initial and ongoing physical therapy evaluations on new patients and re-evaluations as indicated.

• Establish, revise and evaluate a plan of care which is appropriate to problem identified and involve the patient/family, that includes appropriate medical and treatment diagnosis, short and long-term goals, prognosis, results of evaluation, resident goals and discharge planning.

• Complete and accurate notes should be written, and appropriate daily billing performed electronically compliant with professional standard of care

• Participate in interdisciplinary team meetings/trainings/conferences.

Requirements/ Qualifications

• Current physical therapy licensure in the state of Maryland)

• 1 Year of experience *preferred*

• Bachelor's Degree Required.

• Master's degree *preferred*

• Driver’s License credentialed from the Maryland Department of Motor Vehicles preferred.

Compensation

Compensation varies depending on experience

Pay: $80.00 - $90.00 per hour
