We are seeking a LSW to join our team. The successful candidate will promote excellent health care using strong communication and interpersonal skills to support patients and families; work with the multidisciplinary team including physicians, nurses, utilization review, other professionals, and staff; and coordinate care transitions with patients, families, and community resources. The successful candidate will also feel comfortable handling a diverse range of duties, including discharge planning for Med Surg, ICU, Emergency Department, Clinic, and Inpatient Mental Health.
The qualified candidate will possess knowledge of psychosocial needs of patients, families, community resources, family systems, advance care planning, third-party payors and patient rights. Experience in a hospital setting preferred, knowledge of mental health helpful. Minnesota LSW required or pending.
Job Summary:
The social worker provides psychosocial assessment that includes social, psychological, emotional, financial, and spiritual needs. Works with interdisciplinary team member to coordinate care and resources needed as well as planning for any discharge needs and services. Is an advocate for patients.
Essential Duties & Responsibilities:

  • Complete social services assessments which include social, psychological, emotional, financial, and spiritual needs of the patients.
  • Assesses age specific needs based on knowledge of the growth and development and life-stages.
  • Develops social service action plan/discharge plan that is in collaboration with the interdisciplinary team as well as the patient and/or representative.
  • Completes all documentation per regulatory requirements.
  • Assists patients with any financial resources needed.
  • Assists with admission status reviews and concurrent continuing stay reviews in Inpatient Mental Health (IPMH).
  • Collaborates with community agencies to ensure appropriate resources are available.
  • Collaborates with county agencies to provide appropriate documentation for civil commitments as needed (i.e exhibit A, petition for judicial commitment, etc.) (Inpatient Mental Health).
  • Provides community/staff education as assigned.
  • Assist with clarification on policy and procedures.
  • Is a patient advocate.
  • Provides crisis intervention services as needed or requested.
  • Participates in patient care conferences and/or family meetings.
  • Facilitates psychoeducational group on Inpatient as needed.
  • When working on Inpatient Mental Health, gathers collateral information from families and/or significant others.
  • Performs all other related duties as assigned.

Education, Training or Degree Required:

  • Bachelor's degree in social work is required. In lieu of BSW, equivalent relevant experience in areas of social practice is required.

License/Registration/Certification Required: (will be primary source verified by HR)

  • Minnesota Social Work licensure (LSW) required.

Experience and Skills: (indicate preferred or required)

  • Previous discharge planning in hospital setting, social work experience in long-term care or hospice is preferred.
  • Experience with and/or knowledge of Advanced Directives preferred.
  • Knowledge of psychosocial needs of patients, families, community resources, referral process, family systems, third-party payor, and patient rights is required.
  • Basis knowledge of computer skills i.e. e-mail, word, or medical/clinical software is required.

Additional Information:
Position will work approximately every 4th weekend and a rotation of holidays.
