Job Description :

Ward Memorial Hospital is seeking a full time Case Manager/Community Liaison Specialist. The right candidate will work to facilitate patient care by assessing patient needs, evaluating treatment options with medical personnel, coordinating care and tracking patient progress. Experience in health care marketing or social services. No license required. Interested candidates may submit a resume.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Focuses on care provided by skilled care team members to patients.

  • Identifies needs to facilitate quality care delivery.
  • Concurrent review of patient’s medical records to establish criteria and confers with provider and other members of the healthcare team to determine criteria concerning admissions, treatment, and length of stay or patients.
  • Communicates and negotiates with payor sources based on the needs of the patient.
  • Formulates an individualized plan of care appropriately delegating/coordinating tasks and duties to care team members.
  • Utilizes sound knowledge base for decision making and establishing priorities of patient care.
  • Implements and coordinates care in a knowledgeable, skillful, and consistent manner working with physicians, nursing, rehab and other skilled care providers.
  • Initiates utilization management with Utilization Management Software.
  • Provide required information to Third Party Payers, Medicare or Medicaid for continued stay review and authorizations for coverage.
  • Complies with established Standards/Procedures/Protocols
  • Drives the transition of care throughout the patient experience.
  • Coordinate referrals and appointments for outpatient services.
  • Evaluates effectiveness of care given by health care team members, including self, as reflected in the documentation of patient’s progress/responses, communication skills and problem identification and resolution.

Job Type: Full-time

Pay: $23.00 - $24.00 per hour
